  COMPLIMENTARY WRITTEN PRESCRIPTIONS

Written Prescriptions are available for clients to submit directly to third party pharmacies of their choice

Written prescriptions are available during in person appointments, via local pick up at Soul Puppy or by mail

  OUTSIDE THIRD PARTY PHARMACY  REQUEST  & REFILL FORM 

Please submit the required online form in the above link only if you wish for Dr. Brown or remote staff to phone in your pets prescription to an outside third party pharmacy. 

There is a $22 fee per call to account for doctor and remote staff pharmacy communication time. Complimentary written prescriptions are available as stated above

Email requests without the required form will not be processed. Dr. Brown and remote staff will only be able to answer all of the pharmacists questions if this form is properly submitted due to the nature of this practice

Note: the above fee is waived if a prescription medication is not available in MYVETSTOREONLINE such as certain controlled or highly monitored drugs or if a medication needs to be compounded at a compounding pharmacy because an FDA approved drug is not available for the patients weight
