Please submit the new client request form (required) below for the service option that best suites your pets needs (step one) AND immediately go to step two. Both steps are required for request consideration
An email from scheduling@brownvetservices.com will be sent within 1 to 3 business days acknowledging receipt of form submission. All required fields must be filled for forms to process correctly. If an email is not received then your form was not processed or there was an error with the email provided on the form
Please have medical records (required) including rabies & core vaccine history emailed directly from veterinary offices to records@brownvetservices.com by calling clinics directly to release this information to Dr. Brown
Requests are held at this step until medical records are received for review for safety purposes
A status email update will be sent once BOTH the request form & medical records are received & reviewed.
Request approval is at doctor discretion
Pets with a history of aggression and/or in need of sedation for handling will not be accepted for safety purposes
Dr. Brown reserves the right to refuse service at her discretion
Dr. Brown is limited in the number of patients that can be seen daily due to the logistics of scheduling a mobile practice in multiple cities around the valley
Deposits are required for approved requests only by email invitation & instruction (step three)
Deposits will be deducted from invoice total or applied to cancellation fees if less than 48 hours notice provided
Note: any unauthorized deposits received will be charged both a reversal and transaction fee and request will be automatically forfeited.
Appointment availability & scheduling details (step four) are reserved for active patients & preapproved new client requests with authorized paid deposits.
APPROVED requests with AUTHORIZED paid deposits will be emailed appointment calendar offerings for the soonest available appointment based on type of appointment requested & location.
Offerings are held for 24 hours before being offered to the next patient on the wait list.
Due to the nature of this practice Dr. Brown & remote support are not available on demand
