Please have your pets COMPLETE MEDICAL RECORDS with doctor notes, diagnostic test results if completed and RABIES & CORE VACCINE HISTORY emailed directly from your pets current and/or previous veterinary offices to records@brownvetservices.com-REQUIRED

Note: Many veterinary clinics require pet owners to authorize release of this information

Please phone the appropriate vet offices directly to avoid processing delays

All requests are waitlisted at this step until medical records are received for review for safety purposes
