Please have your pets medical records with doctor notes (required) including rabies & core vaccine history (required) emailed directly from your pets current and/or previous veterinary offices to records@brownvetservices.com. 

Note: Many veterinary clinics require pet owners to authorize release of this information

Please phone your pets current and/or previous vet offices directly to avoid processing delays

All requests are waitlisted at this step until medical records are received for review for safety purposes
